DOT Physical Exam Checklist:

  1. Provide accurate and complete health history; honesty is crucial.

  2. Discuss health history and medications with the Examiner.

  3. General physical observation and weight measurement.

  4. Check blood pressure and pulse.

  5. Conduct vision acuity, peripheral vision, and eye exams.

  6. Assess hearing and examine ears.

  7. Auscultate lungs and heart.

  8. Observe and palpate the abdomen.

  9. Perform orthopedic assessment of spine and extremities.

  10. Conduct urinalysis for glucose, protein, blood, and specific gravity.

  11. Review any additional required testing or documentation.

     After successfully completing the DOT Physical exam, drivers will receive their medical certificate and necessary forms.

What To Bring To a DOT Physical Exam:

  1. Driver’s License: Ensure you have a valid driver’s license for identification purposes.

  2. Medical Records: Bring any relevant medical records, especially if you have existing health conditions or are under medical treatment.

  3. List of Medications: Provide a list of current medications you are taking, including dosage and frequency.

  4. Eyeglasses or Contacts: If you use corrective lenses, bring them along for the vision acuity exam.

  5. Hearing Aids: If you use hearing aids, bring them for the hearing assessment.

  6. Health History Information: Be prepared to share your health history, including past surgeries, medical conditions, and any recent health concerns.

  7. Payment: Please present the barcode you received via text and/or email at the clinic when you arrive.  The barcode is your verification of payment for the exam.

  8. Medical Certificates: If possible, bring any previous medical certificates or documentation related to your last DOT physical.

    Being prepared with these items will help streamline your DOT physical exam process.
